Get a Copy of Your CNA License
Certified Nursing Assistants (CNAs) must carry a CNA license issued by their state to work with patients in health care facilities. Many states issue copies of your CNA license that you can keep for yourself or present to potential employers as proof that you are a trained and licensed CNA. The process for obtaining a copy of your CNA license differs slightly from state to state.
Instructions
1. Contact the nurse aide registry for your state by phone or mail. Ask your employer for information on contact the nurse aide registry in your state or obtain a list of nurse aide registries by state on line. (See Resources.)
2. Ask the registry for any necessary forms you need to fill out to obtain a copy of your CNA license. If your state does not issue hard copies of CNA licenses, ask for a phone number that potential employers can call to verify your license.
3. Fill out all of the information completely and correctly on the form to get a copy of your CNA license. Do not leave any required spaces blank. Mail the form to the address specified on the form and include any necessary payment. States that require a fee to obtain a copy of your CNA license will specify what types of payment are acceptable.
